1. Services Provided

Douglas Family and Animal Chiropractic, led by Dr. Shelli Douglas D.C., offers chiropractic care for families and animals. The services include, but are not limited to, spinal adjustments, wellness consultations, and other related chiropractic therapies.

2. Appointment Scheduling and Cancellations

Appointments must be scheduled in advance. We reserve the right to charge a cancellation fee for any appointments canceled with less than 24 hours' notice.

Repeated cancellations or no-shows may result in the requirement of a non-refundable deposit for future appointments.

3. Payment Terms

Payment is due at the time of service. We accept cash, credit cards, and certain insurance plans.

It is the patient’s responsibility to confirm with their insurance provider whether chiropractic care is covered under their plan.

Any outstanding balances must be paid in full before additional services can be rendered

4. Animal Chiropractic Care

All animal chiropractic care is provided under the supervision of Dr. Shelli Douglas, who is certified in animal chiropractic.

Owners must provide a signed consent form acknowledging that animal chiropractic is an adjunctive therapy and not a substitute for veterinary care.

Any existing health issues or conditions must be disclosed prior to treatment.

5. Health and Safety

Patients are required to provide accurate and complete health information.

If you are experiencing symptoms of a contagious illness, please reschedule your appointment.

All animals must be on a leash or in a carrier while on the premises.

6. Privacy Policy

Your personal and health information is protected under HIPAA regulations.

We do not share your information with third parties without your explicit consent, except as required by law.

7. Liability Disclaimer

Chiropractic care is a safe and effective way to manage pain and promote wellness; however, results can vary by individual.

Douglas Family and Animal Chiropractic is not responsible for any adverse effects resulting from services rendered.

By agreeing to treatment, you acknowledge that you understand the risks associated with chiropractic care.

8. Amendments

We reserve the right to modify these terms and conditions at any time. Any changes will be posted on our website and will take effect immediately.

9. Governing Law

These terms and conditions are governed by the laws of the State of Georgia. Any disputes arising from these terms shall be resolved in the appropriate courts located in Rome, GA.
